Residential Care Home for Adults with Learning
Disabilities and Autism
Specialist Support for Young Adults with Autism and Learning Disabilities
New Leaf Haven Homes provides specialist home support for young adults with autism and learning disabilities who require structured, person-centered support to safely transition into adulthood and community based living.
We recognise that no two individuals are the same or have the same needs, preferences or aspirations. Each person we support receives an individualised package of care and support developed around their assessed needs, communication style, sensory profile and personal goals. Our support planning is informed by Positive Behaviour Support (PBS) principles, ensuring that strategies are tailored to the individual to promote emotional regulation, reduce anxiety and enhance the quality of life.
We support individuals who may be transitioning from residential education, at risk of placement breakdown, or requiring a bespoke, PBS led placement to prevent more restrictive care settings.
As a service registered with the Care Quality Commision (CQC), our model is designed to promote independence and reduce the reliance on restrictive practices and enable individuals to develop the skills required for sustainable, long term living within their communities. We deliver support using the Active Support model, ensuring individuals are meaningfully engaged in their daily lives and are enabled to participate as independently as possible in all aspects of their care and support and community involvement.
Our low stimulus, autism-informed environment supports individuals with sensory and communication needs offering consistent routines and skilled staff, trained to deliver personalised support that reflects each individuals strengths, interests and long-term outcomes.
We work in partnership with Local Authorities, Intergrated Care Boards and multidisciplinary professionals to design and deliver tailored support packages that evolve with the individual as their independence and confidence develops.
