Support is provided respectfully and sensitively, promoting dignity and independence at all times.

This may include:

• Assistance with personal hygiene and self-care
• Support with dressing and maintaining personal appearance
• Encouragement to complete tasks as independently as possible

 

Daily Living Support

We support residents to participate actively in everyday home life, helping to build confidence and practical living skills.

Support includes:

• Establishing and maintaining daily routines
• Meal planning and preparation
• Managing personal bedrooms and shared living spaces
• Developing skills required for greater independence

Support levels are tailored to each individual and reviewed regularly to reflect changing needs.

Life Skills Development

Where appropriate, people are supported to develop skills that promote independence and confidence in adulthood.

This may include:

• Building confidence in decision-making
• Developing communication and social skills
• Learning practical daily living skills
• Working towards individual goals identified through care planning

Progress is reviewed regularly to support continued personal development.

Care Planning and Reviews

Each person we support has an individualised Care and Support Plan developed with their involvement and, where appropriate, in partnership with families and professionals.

Care plans:

• Reflect assessed needs, preferences and personal goals
• Include risk assessments and Positive Behaviour Support strategies where required
• Are reviewed regularly to ensure support remains appropriate and effective

Community Integration Programmes

We believe meaningful lives are built through connection, opportunity and participation within the wider community.

People we support are supported to:

• Access local amenities, activities and services


• Develop social confidence and relationships


• Participate in group and individual community activities


• Build independence and reduce social isolation

Our approach encourages residents to engage with community life at a pace that feels safe and comfortable for them.

Health and Appointment Support

While New Leaf Haven Homes does not provide nursing care, we ensure that people are supported to maintain and manage their health and wellbeing.

This includes:

• Supporting attendance at GP, hospital and healthcare appointments
• Safe storage and administration of medication in line with care plans
• Monitoring wellbeing and reporting or escalating concerns appropriately
